How to stop stale context from misleading your AI agent
Stale context is information that was once useful but no longer reflects the current task or business state.

Stale context is information that was once useful but no longer reflects the current task or business state. Prevent it by tracking effective dates, source versions and superseded decisions. A timestamp on a summary is not enough if the facts inside it were copied from older records.
Distinguish written from effective
Imagine a policy summary created today from a document retired last month. The summary is new, but its content is old. An agent sorting only by file modification time may give it more authority than the current policy.
Store the source version and effective date with consequential facts. When the original changes, summaries and indexes need a refresh or a visible stale marker.
Define freshness by consequence
A brand writing preference may tolerate a long review interval. An inventory count may need a live lookup. A contract status should come from the authoritative record at the time of the decision. Use different freshness rules rather than assigning every fact the same expiration period.
- Identify what can change between runs.
- Decide which actions require a fresh lookup.
- Mark cached information with its observation time.
- Define the response when a fresh source is unavailable.
- Preserve the previous value as history, not current truth.
Test a changed fact
Run a synthetic task, update a key source and repeat it. Check whether the agent notices the new version. Then make the source temporarily unavailable. The correct response may be a partial answer that identifies the stale evidence, rather than silently relying on it.
Do not treat a failed lookup as proof that nothing changed. Unavailable coverage is a state of uncertainty.
Make updates easy to trace
Include a short source note beside operational summaries: current as of this observation, based on this record. That gives a human a way to judge whether the information is fit for the next action. Freshness becomes part of the decision instead of a decorative date at the top of a page.
